If your pet is a hider or anxious, please put your pet in the bathroom or in a carrier. We don't want to stress out your pet more than necessary. If the Technicians have to chase or locate your pet there will be additional charges for the extra time required. *

This includes if the Tech has to move around any furniture, pet is hiding under bed etc...

IMPORTANT!!! As a mobile service we always give a 2 HOUR range for arrival. We will do our best to arrive at your appointment within the first hour, but due to traffic etc we may arrive anytime within the two hour range. Please confirm you have read and understand our arrival time. *

EXAMPLE: If your booked appointment slot is 12:00- 2:00 PM, we will do our best to be there ANYTIME between 12-1 but may arrive anytime after 1. We will always call you if we will be arriving after the first hour to let you know.

By confirming/booking this appointment with Monarch Vet Tech Services Inc., I consent to my pet being treated/serviced by experienced staff of Monarch Vet Tech Services. I understand I am required to give 48 hours minimum notice of cancellation prior to my appointment day or 50% (minimum) of my quoted total will be charged. Failure to give a minimum of 72 hrs notice of cancellation for appointments booked with two Vet Technicians will result in being charged a minimum 75% of my quoted total. All outstanding payments/invoices will be sent to our collections agency. I accept and consent to the terms and conditions as stated above. I agree to hold Monarch Vet Tech Services Inc., it’s owners, operators, contractors, officers and directors harmless from any damage, loss and/or claim, arising from any condition of the pet(s) during or after the service. It is also further understood and agreed to the terms of this agreement can change at any time, without notice, and will overwrite any and all prior contracts. I also consent to being sent promotional emails which I can unsubscribe from at any time.
