I ( full name and contact info provided below), give full consent to Monarch Vet Tech Services to tend to the needs of my pet(s) in my home for the dates indicated below and all future dates of care. In case of a medical emergency, I give my full consent for Monarch Vet Tech Services to seek medical care for my pet(s) using my preferred Veterinarian indicated below or if necessary by an after hours medical emergency facility and agree to pay all dues/fees required upon my return.
I acknowledge that:
All prices are subject to HST.
All appointments performed on any stat. holiday will be charged a 100% surcharge of the regular appointment fee. This applies only once per stat. holiday.
We currently only offer once daily visits for Toronto residents only.
Access To Your Home:
We do not keep keys on file. All pet sitting clients must have either a working digital lock with a code ( please be sure to have well charged batteries in before you leave) or have 2 spare keys, one key secured in a lockbox located on the property and one key left with a close and trusted neighbour or concierge.
We offer lockbox rentals for a small additional fee, please inquire with receptionist.
Providing Pet Care Instructions:
If you would like an in person meet and greet we have that option for an additional $50.
Unless you have booked an in person meet and greet with one of our team members, we will ask that you send us some photos or a very short video clip ( 30sec max) to be sent via email to our receptionist detailing pet care instructions. Use your best judgement of important things we should know.
To complete your pet(s) care instructions we will request the following ( if they apply) in the form of photos or short video clip ( sent via email once we confirm your appointment):
-Location of your key lock boxes.
-How to navigate alarm system and codes if needed.
-Location of your pet(s) food, meds if needed, feed dishes, litter pans (including scoops and garbage bags) treats, garbage bins etc…
For cats: If your cat is shy, please let us know where their favourite hiding spots are throughout your home in case we have trouble locating them. We need to at least visualize that they are ok at each visit.
Other important information such as feeding amounts, emergency contacts etc. can be provided on the consent form below.
Cancellations: If pet sitting is cancelled on any statutory holiday (our busiest times), you will be charged for the services that were booked for that day (including additional stat. holiday(s) fees) unless a minimum of 30 days notice of cancellation is given. Please allow for 48 hours notice of cancellation or 50% (minimum) of your quoted total will be charged.
There are additional fees for local pick ups/deliveries if supplies are needed while you are away like litter, food, medications etc. If paid parking or other services are required, the additional amounts will be added to your final invoice.
Full payment is required before you leave unless specified otherwise. Payments can be made via:
